Payment
I accept cash, check, credit/debit cards and HSA/FSA cards.

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?

• What is my deductible and has it been met?

• How many sessions per year does my health insurance cover?

• What is the coverage amount per therapy session?

• Is approval required from my primary care physician?

In Network Provider
I currently accept Optum(United Health), Select Health, and PEHP. I am also in the process of accepting other insurances- in the meantime you can contact your insurance to find out about coverage under your out-of-network benefits. I am able to provide you with a superbill to submit to your insurance for reimbursement.

**I cannot guarantee insurance reimbursement at any time. You will be responsible for full payment up front.

Cancellation Policy
If you do not show up for your scheduled therapy appointment you will be charged the appointment fee in full. If you cancel your appointment within 24 hours of the scheduled appointment, you will be charged a $50 late cancellation fee.

Note: Even if you are using your insurance, insurance does not cover late cancellation fees or missed session fees. You will be responsible for the late cancellation or missed session fee in full.
